09-10 Rec Uniform Order Form - Bring this form to Guilford Sporting Goods to size and order our NEW SCOG UNIFORM. Old uniform style has been discontinued.
Mail In Recreational Registration Form * - 2009-10 Mail In Registration From - please fill out the form with a check to register for recreational soccer. Mail form to P.O. Box 21, Guilford.
MEDICAL RELEASE FORM * - All Parents must fill out a medical release form for each player prior to the start of the season.
PARENT & PLAYER CONDUCT FORM * - Parent Recreational Guildelines - Attached are general rules and policy regarding recreational soccer. Parent, Player and Coaches conduct along with the basic structure of our recreational soccer programs.
SCOG COACHES CODE OF CONDUCT - The Coaches Code of Conduct is to be read and signed by every coach within the SCOG's club.
 

Premier Forms

Expense Reimbursement Form - Form to submit expenses incurred by SCOG coaches related to tournaments.
Mail in Premier Registration Form * - This form is required for all players on a SCOG Developmental Premier/Comp or Premier team.
Mandatory Team Meeting Form - Letter to coaches notifying them they must hold a mandatory team meeting at beginning of each season.
Premier Coach Incentive Form - Form to be filled out by SCOG Premier Coaches to receive additional stipends for time in club and special licensing.
Premier Uniform Order Form 
Team Meeting Recap Form - Form to be filled out by coach or manager following any team meeting. To be submitted to Travel Director upon completion.
 

Financial Assistance Form

Financial Assistance Application - The Soccer Club of Guiford's mission is to provide the opportunity to every child in Guilford, of appropriate age, the ability to participate in our programs. If you find your family in need of financial assistance and question whether or not you can afford our soccer programs, we may be able to help. Please download and fill out the "Financial Assistance Application", mail to P.O. Box 21 in Guilford and we will consider your request. No player may participate until notification.
