SCOG Tournament Request Form

1. All SCOG Travel Teams must complete this form. One Request per Team. Even if you are not partaking in a tournament you must fill this out and enter No Tournament in the Tournament Name.
2. Teams are responsible for registering but not paying for their tournament.
3. Team Personnel will provide to the club Treasurer the login instructions to make electronic payment or mailing instructions to make check payment.
4. Club Treasurer will make payment for all tournaments.
